Cmax
Peak plasma concentration
Cmax is the maximum plasma concentration a compound reaches after a single dose. For SC-injected peptides, Cmax typically arrives 1–4 hours post-injection; for slow-release modifications (acylated, DAC-conjugated, depot formulations) Cmax can be hours to days post-dose.
Cmax matters for both efficacy and safety: it sets the upper bound of receptor occupancy and is often the timepoint at which dose-limiting side effects appear. For GLP-1 agonists, Cmax-related GI side effects (nausea, delayed gastric emptying) are why the formulations are designed with smoothed PK profiles.
Cmax / Cmin ratio (peak-to-trough) is a useful descriptor of dose-profile flatness. A low ratio means smooth, steady coverage; a high ratio means pulsatile exposure.
